What do increasing pollution levels mean for sore throat – a narrative review

Air pollution is a growing global concern with well-documented effects on respiratory health. While much attention has been given to lower respiratory conditions whereas upper airway symptoms—particularly sore throat—has received comparatively less focus. This is especially relevant considering the ever-rising pollution levels globally in recent years.This narrative review explores the relationship between exposure to environmental pollutants, and the incidence and mechanisms of sore throat. It identifies key pollutants involved, affected populations, and potential mechanisms.Website searches (WHO, State of Global Air). Embase search 1: – Search string: 'pollution'/exp AND 'air pollution'/exp AND ('sore throat'/exp OR 'pharyngitis'/exp). – Date limit: 2010-2025. – Population: Newborn – 80+. – English language only. – In humans only. – Peer- reviewed. – 155 hits, 20 were deemed as relevant. Embase search 2: – Search string: 'pollution' AND ('sore throat':ti OR 'pharyngitis':ti). – Date limit: None. – Population: Newborn – 80+. – English language only. – In humans only. – Peer reviewed. – 11 hits, 7 were deemed as relevant. Studies that focused on infectious causes of sore throat, and on respiratory disorders that do not include sore throat were excluded.Pollution is increasing in recent years, with highest levels of the population being exposed above recommended limits of PM2.5 in Asia and Africa. Pollutants such as PM2.5, NO₂, SO₂, and O3 contribute to throat irritation through inflammatory and neurogenic mechanisms. Vulnerable populations including children, the elderly, and those with pre-existing conditions experience heightened symptoms.As pollution levels continue to rise globally, the incidence of sore throat is becoming an increasing health concern. Providing effective symptomatic relief can play a pivotal role in reducing the burden on overstretched healthcare systems by empowering individuals to manage mild symptoms through self-care. This is particularly critical in developing markets, where access to timely medical advice may be limited.Air pollution often contributes to sore throat. This is of particular concern considering exceeding levels of pollution in various regions globally. Future research should aim to build a clearer picture and inform clinical practice.
