The Role of Point-of-Care Ultrasound (POCUS) in Community and Home Care

Due to its portability and accessibility, point-of-care ultrasound (POCUS) has significant potential for application in primary care settings. POCUS offers a portable, radiation-free imaging modality that can support timely clinical decision-making.To review current evidence on the use of POCUS in community and home care settings, and to evaluate its applications, benefits, and limitations relevant to primary care practice.A comprehensive literature search was conducted across six databases in April 2024. Studies involving the use of POCUS in home or community settings for diagnostic purposes, monitoring, procedures, or clinical management were included. A total of seven studies and five case series or reports met inclusion criteria.POCUS was applied across diverse community settings by physicians, nurses, and specialists. Key uses included screening and diagnosis of deep vein thrombosis, heart failure, ascites, pleural effusion, pneumonia, and intra-abdominal pathology. POCUS supported clinical decisions such as fluid assessment, dyspnoea evaluation, urinary retention assessment, and medication titration. It also facilitated bedside procedures including paracentesis and nasogastric tube placement, with high reported sensitivity and specificity. Emerging evidence showed promising roles for artificial intelligence (AI) integration, enabling accurate assessments by operators with limited training.Findings demonstrate that POCUS enhances accessibility to diagnostic imaging in community settings, improves triage, and supports real-time decision-making for patients with limited mobility. It enables safe delivery of bedside procedures and may reduce unnecessary hospital visits. However, operator dependency remains a key challenge, necessitating structured training, competency assessments, and clear clinical frameworks to prevent overdiagnosis. AI integration shows promise in reducing variability and supporting less experienced users, but further validation is needed before large-scale implementation.POCUS is a powerful tool with numerous potential applications in community and home care settings. Current literature highlights only a fraction of its capabilities, with many applications still underexplored. There is considerable potential for future research to expand the use of POCUS in home care, as well as to develop training programs for its application and integrate AI to enhance diagnostic support.
