Across many countries, primary care teams are experiencing rising demand linked to stress, anxiety, sleep disturbance, low mood and lifestyle-related conditions. Digital self-management tools can help patients build healthier routines, improve emotional wellbeing and reduce reliance on reactive care. This workshop introduces practical, transferable strategies for implementing digital wellbeing tools in a wide range of international primary care settings. Globally, wellbeing-related concerns are among the most common reasons patients seek primary care. These issues often coexist with multimorbidity, social stressors and chronic conditions, creating frequent appointments and placing sustained pressure on clinical teams. Many systems have limited capacity for structured psychological support or behavioural interventions, making it difficult to offer continuity between consultations. Digital self-management tools offer a scalable means of supporting patients outside traditional appointments through personalised prompts, behavioural nudges and structured reflection. They can strengthen patient autonomy, reduce avoidable consultations and shift care from reactive to preventative without requiring high-resource infrastructure. Adaptable onboarding, hybrid support for digitally excluded groups and integration with existing workflows make these tools viable across low-, middle- and high-income settings. This workshop focuses on how primary care teams can safely and effectively implement these tools in their own context.Participants will be able to: 1. Understand essential components of digital self-management tools. 2. Identify suitable patient groups and referral criteria. 3. Integrate tools within primary care workflows. 4. Support digital inclusion through structured onboarding and hybrid pathways. 5. Adapt implementation strategies to varying technological and cultural environments.Short presentations on behavioural change, patient activation and workflow integration.
