Severe acute headache with neurological impairment in a young adult: a Primary Care challenge

Acute neurological symptoms often present first in Primary Care, requiring decisions under uncertainty and with limited tools. This case illustrates how careful anamnesis and a focused neurological exam, coupled with an equity-driven referral strategy, enabled early management of a potentially severe central nervous system (CNS) condition.A 24-year-old man attended Primary Care after waking with a sudden, severe headache, photophobia, nausea, myalgias, neck stiffness, and, critically, inability to move or feel both lower limbs (transient paresis). Focused examination confirmed inability to walk/stand and reduced lower limb strength. The constellation of red-flag symptoms suggested severe CNS pathology (e.g., meningitis, subarachnoid hemorrhage). Despite diagnostic uncertainty, the presence of danger signs prompted immediate and urgent referral to Emergency Services, prioritizing patient safety and equitable access to higher care. In hospital, neurological deficits partially normalized, but it was decided to perform a lumbar puncture. The results were decisive, showing lymphocytic pleocytosis. A diagnosis of acute lymphocytic meningitis was established.The originality lies not in the final diagnosis, but in the process: managing a severe, yet transient, neurological deficit alongside meningeal signs in Primary Care. This case highlights that Primary Care’s crucial role is detection (recognizing severity) over diagnosis, promoting a safety-first, equity-driven referral model often overlooked in the literature.The key lesson is that the most critical action was the timely recognition of danger signs and urgent referral, not diagnostic accuracy. Physicians must be cautious when severe, acute, and transient neurological signs appear. Future practice should reinforce training on recognizing low-incidence, high-risk constellations that mandate urgent referral. Post-discharge follow-up in Primary Care is essential for completing the etiological work-up (e.g., serologies, IGRA) while awaiting the neurology appointment.This case validates Primary Care's role as a critical filter in the health system. It demonstrates the superior value of clinical judgment and meticulous history-taking when advanced diagnostic tools are unavailable.This case underscores Primary Care’s responsibility to ensure rapid, fair access to emergency evaluation when red-flag symptoms arise. Accurate early detection was the key to achieving a safe clinical outcome, confirming Primary Care as the decisive gateway for managing time-sensitive CNS pathology.
