Reviving Ancient Ayurvedic Procedures for Modern Pain Management: Clinical Reflections on Agnikarma(therapeutic cauterization) and Viddhakarma(controlled micro-needling)

Context: Ayurveda, the science of life, describes comprehensive methods for maintaining health and managing disease through medicinal, surgical, and parasurgical approaches. Among these, Agnikarma (therapeutic cauterization) and Viddhakarma (controlled micro-needling) are classical interventions indicated in various musculoskeletal, neurological, and pain-related disorders. Case 1: A 45-year-old male with bilateral knee osteoarthritis (VAS 7/10) underwent Agnikarma using a heated Suvarna Shalaka (gold probe) on tender points. Pain reduced to 2/10 immediately and to 1/10 after five days. Case 2: A 45-year-old obese female with anosmia, frontal headache, and joint pain received Viddhakarma using a 26G needle at nasal and eyebrow points, followed by Agnikarma on painful joints. Complete relief from headache and joint pain was achieved with partial restoration of smell. Case 3: A 65-year-old female with radiating shoulder pain to right hand,(VAS 8/10) was treated with Viddhakarma on 2 fingers above Kshipra marma and 4fingers above and below flexed right elbow joint,lateral aspect and Agnikarma on the shoulder girdle. Pain reduced to 1/10 post-procedure immediately.Originality: These procedures offer rapid, sustained pain relief with negligible adverse effects, serving as non-pharmacological alternatives to opioids and NSAIDs. They integrate traditional dosha-based reasoning with modern procedural safety.Agnikarma and Viddhakarma are effective, minimally invasive, and low-cost methods for acute and chronic pain management. Their inclusion in integrative pain clinics could enhance patient outcomes and reduce drug dependency.The observed cases demonstrate significant pain relief through Agnikarma and Viddhakarma, likely via enhanced circulation and neural modulation. These safe, low-cost techniques offer effective non-pharmacological alternatives to NSAIDs or opioids. Their integration into modern pain clinics may strengthen evidence-based, holistic approaches to global pain management.The revival of classical Ayurvedic procedures holds promise for global integrative medicine by combining ancient therapeutic wisdom with modern clinical practice.
