Relationship between cardiovascular risk and daily step counts for community health workers in a family health center

Cardiovascular risk (CR) is a major epidemiological marker associated with clinical conditions of high epidemiological impact which require long-term vigilance and adhesion to treatment. In XXX, community health workers (CHW) are a pillar of primary care, and are professionals with a high demand for long daily walks in order to perform their basic tasks inside the community. Medical literature has emphasized daily step count (DSC) as a factor inversely associated with CR in other populations. Association between CR risk and DSC has not been studied for XXX CHW.The aim of this study was to evaluate a putative association between CR and DSC for CHW in a family health center in XXX.This study was part of an undergraduate extension program. CHW were trained in the usage of the free cell phone applications that quantified CR and DSC, and then scheduled to return 30 days later. Upon return, CHW filled out a form containing every parameter needed to feed the CR calculator, including some measured on the spot. Form also collected DSC for every preceding 14 days, and the average DSC was calculated. Association between CR and DSC were analyzed by linear regression and Mann-Whitney U-test. This protocol was approved by the institutional ethics committee.Participant CHW were n=36; 77,70 % were female; 58,3 % were older than 40 years; 80,5 % were overweighted or obese; 33,3 % had high blood pressure; 27,7 % had moderate to high CR; 63,8 % had DSC higher than 5.000. Statistical analysis found no significant association between CR and DSC, even when CWH were further stratified into subsets.Contrary to expected from current medical literature, no association was found between CR and DSC for CHW. One hypothesis is this group is too small and heterogeneous, not reflecting general population. Another possibility is daily walking is not protective enough from hazards like high blood pressure or obesity, which would demand specific attention.In conclusion, CHW in a family health center in XXX do not seem to get benefit in a simple way from high DSC, emphasizing the importance of comprehensive preventive health care.
