Diabetes mellitus presents a spectrum ranging from classical type 1 (T1DM) and type 2 diabetes (T2DM) to hybrid and atypical forms, including latent autoimmune diabetes in adults (LADA) and maturity-onset diabetes of the young (MODY). Accurate classification in primary care is challenging due to overlapping phenotypes, variable clinical presentations, and limitations of conventional diagnostic tools. Misclassification may lead to suboptimal treatment, delayed intervention, and increased risk of complications.Delegates will gain a better understanding of the following: Epidemiology and global burden of types of diabetes Pathophysiology of T2DM , T1DM and other types Diagnosis pitfalls and dilemmas Emerging role of CGM Early identification & Prevention – ‘a panacea’ for health systemsA narrative review was conducted using current literature from PubMed, MEDLINE, and primary care guidelines. Emphasis was placed on diagnostic criteria, biomarkers, and clinical features distinguishing T1DM, T2DM, LADA, and MODY. Case vignettes and algorithmic approaches were synthesized to illustrate decision-making pathways for ambiguous presentations.T1DM: Typically presents acutely in children or young adults, with hyperglycemia, ketonemia, and positive autoantibodies (GAD65, IA-2, ZnT8). T2DM: Common in adults with obesity, insulin resistance, and gradual onset; often associated with metabolic syndrome features. Intermediate forms (LADA, MODY): Display mixed phenotypes, slower progression of beta-cell failure, and variable autoantibody or genetic markers. Diagnostic tools: C-peptide measurement, autoantibody panels, and genetic testing improve classification but are not universally available in primary care. Algorithms combining age, BMI, clinical features, and laboratory markers improve diagnostic accuracy.Primary care providers often face dilemmas when diabetes does not fit classical definitions. Early recognition of atypical forms is essential to guide therapy—insulin initiation in T1DM or LADA, oral agents in T2DM, and targeted therapies in MODY. Clinical vigilance, combined with selective use of biochemical and immunological tests, can optimize outcomes. Challenges include cost, access to specialized testing, and the dynamic nature of diabetes progression.Accurate classification of diabetes in primary care requires an integrated approach, balancing clinical assessment with targeted investigations. Awareness of intermediate and atypical forms, alongside practical algorithms, enhances diagnostic precision and ensures appropriate treatment initiation, ultimately improving patient outcomes.
