Person-centred care is a core value in general practice. It means focusing on the whole person rather than just their disease, considering individual characteristics, needs, and context. This approach is particularly important for patients with limited health literacy or in unfavorable socioeconomic circumstances, who often face multiple health and/or social problems, like co-morbidities, poverty or chronic stress. Turning this principle into practice is not always straightforward, particularly for those patients who find it difficult to express their concerns and needs. In this workshop, we present evidence on the importance of person-centred care for this group and share practical tools and strategies. Participants will learn about a co-created conversation tool to facilitate meaningful dialogue to understand patients social context.After attending this session, participants will: Understand challenges in person-centered care for patients in vulnerable positions and the importance social context; Identify key aspects to discuss during consultations to gain insight into patients’ context, needs, and preferences. Develop practical ideas on how GPs can better tailor care to these patients and  collaborate effectively within network-based care and the social domain.The first part includes a presentation (20 min) on research into experiences and needs in GP care of patients in vulnerable positions and the importance of person-centered approaches. This is followed by a 10-minute group discussion on shaping personalized primary care from participants’ perspectives. The second part (30 minutes) involves a demonstration of a conversation tool that GPs can use to support meaningful dialogue. Participants will discuss the tool and explore opportunities for daily practice.Participants will gain: awareness of the importance of person-centred care for patients in vulnerable positions practical support to better address health needs and context to apply person-centered care.Patients in vulnerable positions face unique challenges in understanding and  navigating healthcare. Person-centred care is therefore not just a core value but a critical strategy for this group and calls on to GPs as essential care providers to understand these patients’ complex social context. This workshop offers GPs practical strategies and tools to put this into everyday practice.
