PCDE Workshop on Chronic Kidney Disease: A step-by-step, evidence-based, practical approach in primary care

Patients with chronic kidney disease (CKD) and type 2 diabetes (T2D) are at high risk of CKD progression and cardiovascular (CV) disease. Prevalence of CKD in patients with T2D is currently around 40% and continues to grow. The cardio-renal axis represents the complex interplay between the heart and kidneys, two vital organs that are intricately linked in maintaining homeostasis. Management of CKD in patients with T2D aims to preserve kidney function to reduce the risk of end-stage kidney disease, CV events, and mortality. Most of this early detection and intensive management occurs within primary care.Delegates will gain a better understanding of the following: Why is CKD important – the cardio-renal-metabolic interplay Epidemiology of CKD Prevention and screening strategies of CKD Risk assessment in people with CKD Monitoring frequency and early management of CKD Delaying CKD progression and managing its complications When to refer to kidney specialists (including KFRE)The format will be a face-to-face Workshop session , facilitated by 2-3 PCDE members with an expertise on CKD and CVD (cardiometabolism) Program outline Introduction of participants Kick-off presentation Grouping: 6 groups of max. 8 participants, each Presentation of clinical cases (one for each group) Group work Plenary session: group presentation of clinical management of cases Group discussion – using KDIGO PCDE one pager and Kidney-Failure Risk Equation (KFRE)Improved clinical knowledge – CKD diagnosis, staging and progression Better Screnning and Early detection practices : high-risk groups Increased condfidence in management – pillar approach Enhanced care coordination : When to refer , care pathways Evaluation outcomes: Pre/post knowledge scores , commitment-to-change statements and adoption of tools taught in workshopThe CKD primary care workshop improves clinicians’ knowledge and confidence in the early detection, staging, and management of chronic kidney disease, emphasizing the critical role of primary care in slowing disease progression. Participants gain practical skills in interpreting eGFR and ACR results, initiating evidence-based therapies, and providing patient-centered education, while also strengthening understanding of referral pathways and interdisciplinary coordination. Overall, the workshop supports meaningful practice enhancements that promote earlier intervention and better long-term outcomes for patients with or at risk of CKD.
