Neighbourhood Health: A GP-Led Model for Integrated, Proactive, Person-Centred Care

Across the NHS and Europe, health systems are seeking new ways to strengthen primary care, reduce fragmentation, and address widening inequalities. A GP-led at scale provider has pioneered NHS England’s ‘Neighbourhood Health’ model that brings together general practice, community services, mental health, social care, and the voluntary sector to work as one team around local populations. Grounded in the principles of prevention, integration and empowerment, the model offers a practical framework for translating population-health ambitions into daily general practice.Participants will: 1. Understand the core principles of Neighbourhood Health. 2. Explore approaches to building multidisciplinary teams around local populations. 3. Reflect on the GP’s role as connector, facilitator and system integrator. 4. Identify transferable lessons and enablers for neighbourhood-based working. 5. Co-design local adaptations relevant to their own context.0–10 min: Introduction and framing – overview of the neighbourhood model and NHS policy context. 10–25 min: Small-group mapping of participants’ local ‘neighbourhood ecosystems’ 25–40 min: Case discussion – Community Frailty Hub 40–55 min: Whole-group reflection on barriers, enablers, and transferable learning. 55–60 min: Closing summary and take-away toolkit.Participants will gain practical insight into how Neighbourhood Health can be implemented within primary care, including mapping community assets, strengthening multidisciplinary collaboration, and initiating GP-led system integration.Neighbourhood Health demonstrates how general practice can act as the integrator of local health and care systems—shifting focus from illness to prevention, from services to communities, and from fragmentation to shared purpose. The workshop offers actionable, evidence-based learning for clinicians, managers and policymakers seeking to build equitable, person-centred primary care across Europe.
