Natural pregnancy following regular running in a subfertile couple: a practice-based reflection by a family physician

Age-related fertility decline increasingly affects couples worldwide. As a 43-year-old family physician with no prior running experience, I faced repeated assisted reproductive technology (ART) failures with my wife. This personal struggle prompted reflection on whether accessible lifestyle strategies—particularly aerobic activity that even complete beginners can adopt—may support natural conception in middle-aged couples.In 2024, my wife and I underwent three cycles of intrauterine insemination (IUI): one failed to implant and two ended in early loss. With concerns about age-related decline, we began a shared running routine. As a complete novice, I started on June 9th, 2025 with 1-minute treadmill runs at 10 km/h followed by 2–3 minutes of walking, gradually increasing intervals weekly. Over several weeks, I progressed to running 10 continuous minutes and more than 5 km outdoors at a pace of 6′30″/km. My wife, also a beginner, followed a similar gradual protocol at 8 km/h initial bouts, eventually running ~3 km outdoors at 7′30″/km. This routine improved vitality and sleep regularity. Remarkably, my wife conceived naturally in August 2025—two months after beginning the program.This case uniquely illustrates how a middle-aged couple with no prior running experience may achieve natural conception after repeated ART failures through progressive, accessible aerobic training. It highlights a realistic “zero-to-hero” pathway demonstrating that individuals of any age, body size, or fitness level can safely begin running using short bouts, slow pacing, and gradual workload increases.Structured aerobic activity may serve as a low-cost, equitable adjunct during fertility counseling for middle-aged couples. Integrating modifiable couple-level lifestyle strategies into primary care may enhance patient-centered fertility support.Improvements in sleep, metabolic health, and shared behavioral changes likely created a favorable biological environment for conception. The synergistic effect of both partners engaging in progressive exercise may have amplified these benefits beyond what prior ART attempts achieved.Regular running initiated by two previously sedentary adults was temporally associated with natural conception following ART failures. Aerobic exercise may represent an accessible, evidence-informed element of infertility counseling—feasible for individuals of all ages, body sizes, and fitness backgrounds.
