In UK 92% of patient population are registered at primary care practices.  Primary Care is key to deliver assessment, treatment, continuing care, person centred care and preventive medicine to the general population. Primary Care is at the forefront to address health inequalities. NHS services is free and GPs are often the first point of contact for most people accessing health care irrespective of race, colour, sexual orientation, gender identity and economic background. Undocumented people are also able to register with a GP in the UK. In Germany 90% of the population are insured through health insurance companies. Access to Primary Care is available and every patient has a free choice of doctor. There has been a strong trend in the last few years towards Primary care with multidisciplinary (MDT} cooperation. In France primary care is mostly based on the Convention Nationale, with the specific objectives of improving coordination and quality of care, maintaining and improving access to care, respecting the choice of patients. Primary care coverage is universal and undocumented immigrants are covered through the State Medical Aid. Primary care workforce in France is self-employed with a growing tendency towards a comprehensive MDT team.There are different models of care where primary care worldwide has been providing gender care services in a friendly, safe, confidential, non- judgemental and accessible space both in person and online. The provision of care in a MDT setting is key to provide an excellent service with good clinical governance. The model of care has shifted from a psychiatric model, focussing on providing a comprehensive care. In the UK, Germany and France the wider primary care network plays a crucial role in supporting transgender people by providing ongoing hormone therapy prescriptions, routine general medical care, preventive health interventions such as cancer screening, and end-of-life care.
