Improving diabetes screening in a suburban polyclinic

In this country, the mortality rate from diabetes is the highest in the region, with 50.9 cases per 100,000 population annually. In this Polyclinic, serving 38,000 patients, medical staff’s attention was focused on managing already diagnosed cases, with no measures for early detection and prevention. Previous years’ data showed poor screening rates and low population awareness. The project aimed to increase early detection, enhance public awareness, and strengthen involvement and organization of the medical staff.The response involved identifying and noting high-risk groups amongst 35-68 year-olds, introducing a mobilization method through calls by nurses to invite patients for screenings, and providing healthy lifestyle counseling. Obstacles included renovation of the polyclinic (which created accessibility issues, impacting screening numbers), people’s fear of getting tested, or not considering it necessary. These were overcome by providing screening and counseling sessions in accessible rooms.Quality Improvement and team-based care are very new concepts in this country. This project broke from the previous passive provider-dependent approach, ensured increased coverage and improved teamwork. Staff became involved in medical examinations as well as informational and organizational phases.The success of any prevention initiative depends on accessibility and trust. A personalized approach increases participation rates and creates a stronger connection. Community-based mobilization, active phone calls, and awareness-raising activities can become effective tools for the early detection of other diseases. For sustainability, the Polyclinic will establish a permanent screening schedule continuous training of healthcare workers, and ongoing community awareness activities.Screening rates increased from 23% in 2023-24 to 38% in 2024-25, despite two months with low numbers due to building construction. The screenings identified 20 cases of type 2 diabetes, and 18 cases of prediabetes, with some referred to endocrinology, while others were treated by their family doctor, based on state guidelines. Many patients noted that if it were not for this initiative, they would not have been screened, creating new levels of trust in primary health care in the community.Data collection, adherence to planned schedule, and monitoring of results contributed to success. This community-based approach demonstrated that active population engagement can increase participation rates in screening.
