The Learning Health System (LHS) is a transformative model for continuously enhancing quality of care, through ongoing audit and feedback loops using advanced technologies, data integration, stakeholder participation, and alignment with the Quintuple Aim framework. Despite its proven potential, its implementation remains limited in Europe and Canada. Given advancements in information technology, increasing pressures on healthcare systems and growing burden of chronic diseases, there is an urgent need to scale up LHS in primary care.By the end of this workshop, participants will be able to: Discuss barriers and facilitators to the large-scale LHS implementation within interprofessional primary care teams (PCT) Foster strong partnerships with patients and all stakeholders to ensure meaningful engagement Identify how scalability assessment plays a role in achieving greater equity and humanism in primary healthcare within the scaling process using ISSaQ 4.0 to evaluate the scalability of a large-scale LHS across multiple jurisdictions.Interactive strategies, including expectation mapping, small-group discussions, table-based reflections, and plenary exchanges: Participants will engage in practical exercises and collaborative analysis to reinforce learning. Workshop schedule (60 minutes) Opening (5 min): Welcome participants, introduce facilitators, collect participant expectations. Presentation of objectives (2 min): Outline learning goals. Reflective activity (5 min): Discuss LHS attributes and scalability assessment and its importance. Apply a scalability lens to LHS in PHT:  (8 min): Components, users, using principles. Break (5 min) Case study (30 min): Apply ISSaQ 4.0 in groups, analyze results and share insights in plenary.
