Holistic Lifestyle Assessment Revealing Dietary Triggers in Chronic Abdominal Pain and Diarrhea: A Primary Care Case Report

Chronic abdominal pain and diarrhea are common in primary care and often investigated extensively. When tests are normal, symptoms may arise from modifiable lifestyle, dietary, or environmental factors. A holistic assessment of daily habits, food environments, and work routines can reveal overlooked triggers.A 24-year-old woman presented with several months of intermittent abdominal pain and diarrhea. Previous evaluations, including upper endoscopy and colonoscopy, were normal. A comprehensive assessment explored symptom timing, diet, food sources, work environment, stress, sleep, bowel habits, and psychosocial factors. The patient reported being symptom-free on weekends when eating home-cooked meals but experiencing flares during workdays. She frequently consumed high-fat, fried cafeteria meals due to limited time at work. Stress and sleep patterns were unrelated to symptoms. Pain and diarrhea typically began two to four hours after cafeteria meals. A personalized lifestyle plan was created recommending avoidance of high-fat workplace foods and encouraging healthier pre-prepared meals. No medication was prescribed.This case illustrates that persistent gastrointestinal symptoms with normal test results may reflect everyday dietary exposures. The clear link between cafeteria meals and symptom flares represents a situational trigger unlikely to be recognized without structured lifestyle questioning.Chronic gastrointestinal symptoms should prompt clinicians to explore diet and environmental contributors rather than rely solely on biomedical testing. Routine incorporation of lifestyle assessments—including eating patterns, food environments, and work routines—can identify modifiable triggers. Integrating lifestyle counseling into primary care may reduce unnecessary investigations and improve patient outcomes.Diagnostic testing rules out organic disease but cannot capture environmental influences. In this case, symptoms were diet-specific and setting-dependent. Targeted lifestyle modification achieved meaningful improvement. Evidence shows functional abdominal pain and diarrhea often improve with dietary changes, particularly reducing high-fat meals.Holistic assessment is essential in managing chronic gastrointestinal symptoms. Evaluating lifestyle, diet, and environmental factors alongside medical tests enables early identification of modifiable triggers.
