Diabetes Mellitus and Vulvovaginal Disorders

Diabetes mellitus (DM) has a significant impact on women’s health, increasing susceptibility to both infectious and non-infectious vulvovaginal disorders. Alterations in glucose metabolism may disrupt the normal vaginal environment and predispose to recurrent infections.To investigate the association between poor glycemic control in diabetes mellitus and the increased incidence of vulvovaginal infections.Vaginal discharge examination results collected over a one-year period from a clinical microbiology laboratory in the Piraeus area were retrospectively analyzed. Microscopic examination was initially performed, followed by amine testing, cultures for aerobic and anaerobic bacteria, Mycoplasma and Ureaplasma species, and monoclonal antibody testing for the detection of Chlamydia trachomatis.A total of 120 positive cases were identified. Of these, 72 cases involved diabetic patients with fasting blood glucose levels exceeding 140 mg/dL, while 48 cases involved non-diabetic patients. Among diabetic patients, 49 were positive for Candida albicans, 2 for Trichomonas vaginalis, 16 for Gardnerella vaginalis, 1 for Chlamydia trachomatis, and 4 for Mycoplasma hominis. Among non-diabetic patients, 20 were positive for Candida albicans, 5 for Trichomonas vaginalis, 6 for Gardnerella vaginalis, 6 for Chlamydia trachomatis, and 11 for Mycoplasma hominis.The findings demonstrate a higher prevalence of vulvovaginal infections among women with poorly controlled diabetes, particularly fungal infections caused by Candida albicans. Hyperglycemia may alter vaginal pH and impair local immune defenses, thereby facilitating the growth of pathogenic microorganisms.Poor glycemic control is strongly associated with frequent and recurrent vulvovaginal infections. Effective regulation of blood glucose levels may play a crucial role in preventing vulvovaginal infections and improving gynecological health in women with diabetes mellitus.
