Primary Sjögren’s syndrome usually presents with chronic sicca symptoms, while acute sensory or ocular manifestations are atypical and may obscure the diagnostic pathway. When sudden sensorineural hearing loss (SSNHL) precedes other autoimmune features by several years, the connection is easily overlooked. This case illustrates the diagnostic complexity of recognizing Sjögren’s syndrome in a patient with a long interval between an acute auditory event and subsequent ocular and cutaneous inflammatory manifestations.
