Demographic trends and systemic challenges pose threats to the continuity of primary care in XXX. To provide an effective response to these challenges, a shared vision for primary care is essential. The previous vision no longer fully reflected current challenges or emerging technological opportunities. Therefore, XXX, XXX, and XXX collaborated to create a new vision: XXX. General practitioners (GPs) in XXX constitute a heterogeneous group (including locum GPs, practice owners and GPs on salary), with diverse professional expectations. To ensure successful implementation, broad support from all XXX general practitioners was crucial. Therefore, we included GPs in all phases of the process.We used several established scientific methods to formulate the vision bottom-up, including a participatory design that combined qualitative and consensus-driven methodologies. Core values for general practice, as well as a review of relevant literature served as a baseline. We used focus groups and expert interviews to explore perspectives on the future of primary care. With these data, we developed a draft vision. Key components of this draft were validated through a nationwide survey among GPs. During this process, a large group of representatives of GPs and an advisory panel provided feedback at various times. The process contributed to broad consensus, which was reflected in widespread support during the final endorsement.The vision was co-created through consensus-driven dialogue, integrating diverse perspectives. This bottom-up method facilitated a shared vision, with broad support, from the heterogeneous group of XXX GPs.The collaborative development of this vision showed that structured, inclusive participation of GPs strengthens broad support. Such an approach is essential in heterogeneous contexts.XXX primary care has a longstanding tradition of joint vision development by XXX, XXX and XXX. These organizations represent nearly all GPs and regional bodies. This structure enables broad support for shared strategic visions.The co-creation of this vision, through a bottom-up approach involving GPs in every aspect, enhances “fraternity” and support, and therefore future sustainable implementation.
