An injury to the shoulder or something else entirely?

41-year-old woman with no significant medical history came in for an examination due to pain in her right shoulder. The pain appeared suddenly on Friday evening after she had hung up laundry. Due to the severe pain, she was examined at the emergency room on Friday evening, Saturday, and Sunday, with the conclusion being a traumatic strain of the right shoulder. Analgesics were administered with temporary relief. Status presens: patient exhausted and sweaty, she stated that she had been coughing for the last few days, she had a temperature of up to 38.5°C once, right shoulder is sensitive, mobility normal. Auscultation of the right lung weakened from the base to the scapula. A POCT CRP test elevated ( above 200 mg/ml) a POCUS / Point of care Ultrasound/, showed no sliding and extensive effusion of the right lung. Diagnosis: V.s. lung empyema. The patient was transported to the pulmonary department in emergency mode. Excesive empyema of the right lung was confirmed. Although the course was complicated, patient survived and is doing relatively well.The correct diagnosis was made in a very short time, approximately 15 minutes, based on a detailed medical history, thorough clinical examination, and the use of POCT methods – POCT CRP and POCUS.This case study is unique in that, despite the patient being seen several times in the emergency room, her complaints were downplayed and she was misdiagnosed. I have known the patient for more than 20 years, so I knew at first glance that her condition was serious.The use of POCUS imaging helped me convince the patient, who believed she "only" had a pulled shoulder, to undergo acute hospitalization.POCT devices are not only helpful in determining an early diagnosis, but also helps save lives.How are POCT devices/methods used in your country?Since 2024, general practitioners in XXXX are able to use a new method within POCT devices, the point-of-care ultrasonography (POCUS). It can be used independently or in combination with other POCTs. POCT- CRP, Troponin T, Nt-pro BNP, and D-dimer testing.Combining POCT methods in practice helps both doctors and patients.
