Continuing Education in Health (CEH) promotes evidence-based clinical updating, avoids therapeutic inertia and encourages comprehensive and interprofessional care. The Working Group (WG) for Caring of People with Chronic Conditions of  XXX developed the "Guide for Caring in Hypertension and Type 2 Diabetes (DM2)" to support CEH based on the Chronic Care Model (CCM).Establish a CEH based on its own guidelines for qualifying care in Primary Health Care (PHC).Pedagogical process supported by active learning methodologies and content based on up-to-date scientific evidence, which systematizes and guides the practice of professionals in the care of individuals with hypertension and DM2 in primary health care. Developed in four stages: 1) Survey of learning priorities chosen by professionals through a questionnaire; 2) Planning of activities (themes definition, target public, pedagogical framework, active learning methodologies, teaching strategies, resources, and schedule); 3) Execution of activities; 4) Evaluation regarding participation, satisfaction, quality of the CEH and skills acquired. We applied questionnaires to 250 PHC professionals. The activities were directed to physicians, nurses, nursing technicians, nutritionists, pharmacists, dentists, and community health agents.In 2024, 14 CEH were carried out, with 292 participants. In 2025, were developed 19 CEH, with 579 participants. The chosen themes were: CCM; strategies for guiding lifestyle changes; specific and interprofessional care in hypertension and DM2. The evaluations of the professionals who underwent the CEH highlighted that the active learning methodologies, the "Guide for Caring in Hypertension and DM2" compiling the most up-to-date theoretical references,  and the sequence of CEH were essential for rethinking their practice and optimizing the comprehensive and interprofessional care of people living with these conditions.The CEH reflects the qualification of clinical practice in PHC. The Guide, available through a QR Code, enables quick review. Consolidating the CCM and the good care practices through CEH is challenging. Structuring a theoretical framework that meets the needs of PHC constitutes a differentiating factor in the proposed CEH.The CEH for interprofessional teams in PHC enabled the inclusion of practices and were recognized as important in improving the quality of care for people with hypertension and type 2 diabetes in primary health.
