Blind and Ultrasound-Guided Injection Therapy in Contemporary General Practice

Injection therapy is a widely adopted intervention in the management of musculoskeletal and inflammatory disorders within primary care. Traditionally performed using blind techniques based on anatomical landmarks, these procedures are increasingly supported by point-of-care ultrasound. The growing availability of portable ultrasound devices has expanded their potential role in General Practice, offering new opportunities to improve diagnostic accuracy and therapeutic precision.This work is based on a narrative review of the scientific literature, including clinical guidelines, comparative studies, and case series addressing blind and ultrasound-guided injection techniques. Published evidence was integrated with clinical experience in General Practice to evaluate feasibility, safety, effectiveness, and applicability in routine Primary Care settings.This study specifically focuses on the implementation of ultrasound-guided injection therapy in General Practice, a setting traditionally underrepresented in interventional musculoskeletal research. It highlights the role of the General Practitioner not only as a referrer but also as an active provider of image-guided procedures.Ultrasound guidance consistently improves needle placement accuracy, reduces procedural complications, and enhances patient confidence. However, successful implementation requires structured training, access to appropriate technology, and organisational support. Future integration may redefine the scope of procedural skills in Primary Care.Compared with blind techniques, ultrasound-guided injections demonstrate superior clinical outcomes, particularly in anatomically complex or previously treated sites. While blind approaches remain effective in selected cases, ultrasound guidance provides added value in terms of safety and therapeutic efficacy.Injection therapy remains a cornerstone of musculoskeletal care in General Practice. Ultrasound-guided techniques represent an important advancement, enabling General Practitioners to deliver safer, more precise, and patient-centred care. With adequate training and resources, their integration into Primary Care has the potential to significantly improve clinical outcomes and optimise healthcare delivery
