The WHO First Clinical Guidelines on Tobacco Cessation for Adults highlight that brief advice delivered by healthcare providers is among the most effective interventions to support smoking cessation.We examined the current practices, barriers, and opportunities faced by the regional primary care physicians in providing smoking cessation counseling.In 2022, 45 in-depth telephone interviews were conducted with primary care physicians across three regions of XXX to explore various aspects of smoking cessation counseling.A total of 96% of doctors recognize smoking\u00a0 as a significant risk factor for the development of NCDs. The same portion reported providing smoking cessation counseling, with the frequency of consultations influenced by patient-specific diseases (cardiovascular disease, diabetes, chronic obstructive pulmonary disease, hypertension), symptoms (shortness of breath, cough, high blood pressure, pregnancy), age and smoking history. All physicians can identify smokers through observable indicators, such as smell, hoarse voice, cough, and yellowing of fingers. Most physicians (89%) highlighted smoking as an NCD risk and used tailored question\u2013answer counseling with examples and statistics. Many physicians highlighted the limited evidence on the health effects of newer tobacco and nicotine products. 71% of doctors consistently documented patient&#039;s smoking in medical records and 70% of doctors used this information in clinical decisions, including treatment adjustments, medication choices, and monitoring smoking behavior. Half of physicians reported that patients rarely sought smoking cessation counseling on their own. Self-referral typically occurred when patients\u2019 health declined or they recognized smoking\u2019s impact on disease progression.Since heavy smokers are primary recognized by doctors, they are more likely to receive counseling than light smokers, indicating inequalities in access to care. Limited evidence on the risks of newer tobacco and nicotine products restricts physicians\u2019 ability to provide persuasive smoking cessation counseling.Our study highlights the need for regular training of healthcare professionals in effective, evidence-based interventions to raise awareness of smoking\u2019s impact and strengthen tobacco control.
