Laboratory tests suggested inflammatory activation.CT of the thorax revealed right lower lobe consolidation with air bronchogram, a pleural hydroaerial level compatible with empyema,and bilateral emphysema with areas of ground-glass opacity.Bronchoscopy-guided thoracic ultrasound confirmed a cavitated lesion, and thoracentesis yielded scant serohematic fluid.Due to the small loculated chamber and the patient\u2019s frailty, immediate drainage was deferred.This case is notable for the combination of a cavitated empyema in a laryngectomized,cognitively impaired older adult,initially presenting with nonspecific symptoms easily attributed to baseline status.It highlights how POCUS in primary care and early imaging escalation can change the diagnostic trajectory.Similar reports in the literature usually describe clearer infectious presentations,whereas in this case deterioration was subtle and identified only through attentive longitudinal assessment.Primary care is essential in detecting early respiratory decline,especially in frail patients with altered baseline examinations.Diagnostic coordination between primary and hospital care,including the increasing use of bedside ultrasound,may prevent delays and reduce complications.The case underscores the complexity of decision-making regarding drainage in empyema when frailty and procedural risk complicate management.Multidisciplinary evaluation allowed individualized care while avoiding unnecessary interventions.For family physicians,the case reinforces the importance of functional change,caregiver reports,and maintaining high suspicion in vulnerable populations.Early identification of deterioration in primary care and coordinated escalation of diagnostic testing enabled timely recognition of a cavitated empyema,improving patient safety and demonstrating the fundamental role of primary care in managing complex respiratory infections.<\/p>\n","protected":false},"template":"","class_list":["post-20805","wsa_session","type-wsa_session","status-publish","hentry","description-off"],"_links":{"self":[{"href":"https:\/\/woncaeurope2026.org\/fr\/wp-json\/wp\/v2\/wsa_session\/20805","targetHints":{"allow":["GET"]}}],"collection":[{"href":"https:\/\/woncaeurope2026.org\/fr\/wp-json\/wp\/v2\/wsa_session"}],"about":[{"href":"https:\/\/woncaeurope2026.org\/fr\/wp-json\/wp\/v2\/types\/wsa_session"}],"wp:attachment":[{"href":"https:\/\/woncaeurope2026.org\/fr\/wp-json\/wp\/v2\/media?parent=20805"}],"curies":[{"name":"wp","href":"https:\/\/api.w.org\/{rel}","templated":true}]}}
