Social environment and previous dedication to sports were explored and was encouraged to resume exercise given his strong motivation and family support. Treatment was initiated with semaglutide as there are articles (Ganeshalingam et al., Sass et al., or Manepalli et al.), where it\u00b4s been observed to improve glycemic control in patients with schizophrenia and antipsychotics without worsening psychiatric symptoms and improving adherence to treatment. Within 3 months patient resumed cycling and had considerably modified his habits. However, due to a lack of supply, semaglutide 1mg\/ml subcutaneous once a week was replaced by semaglutide 14mg oral daily dosis, to which patient did not adhere. Nevertheless, tirzepatide was prescribed. Thanks to family support and after considering the economic burden it entailed, as this medication in not covered by Social Security, a significant improvement of HbA1c 5.4% by June 2025 was achieved. Medication was maintained until semaglutide SC could be restarted, upholding excellent control of his diabetes.This case of successful metabolic control was achieved thanks to the combination of pharmacological and psychosocial factors, opting for a holistic approach and seen our patient as a whole, implicating much more than simply medication. Our purpose is to emphasize the importance of including all parts of our patients\u00b4 lives during follow up for a better and complete outcome.As Luciano et al. suggest, continued psychosocial support and exercise promote treatment adherence, helping both glycemic control and mental health in patients with these health problems.In Primary Care lack of time sometimes leads to prioritize pharmacological treatment, overlooking socioeconomic factors.
