Despite its increasing recognition in gerontology and immunology, there is no simple, clinically applicable method for assessing inflammaging in family medicine. Given the central role of primary care in prevention and management of chronic disease, a pragmatic screening tool is needed to identify patients at risk of accelerated biological aging and to guide timely lifestyle-based interventions.To develop and pilot-test a feasible, patient-friendly Inflammaging Risk Score (IRS) designed for routine use in family medicine consultations.A cross-sectional feasibility study was conducted in a family medicine practice in Romania. Adults aged 35\u201375 years were assessed using the IRS, a 10-item composite score integrating anthropometric indicators (BMI, waist circumference), lifestyle factors (dietary quality, sleep duration, physical activity, smoking), stress, digestive symptoms (proxy for gut dysbiosis), and available laboratory markers (Vitamin D and CRP). Each domain was scored 0\u20132 (low\/moderate\/high risk). Primary outcomes were feasibility (completion time, ease of use), patient acceptability, and identification of predominant modifiable risk patterns.Fifty patients were included. IRS completion required 5\u20137 minutes, with 93% patient acceptability and full completion for all non-laboratory domains. The most frequent high-risk domains were: dietary inflammatory patterns, abdominal obesity, poor sleep, and digestive dysbiosis symptoms. Patients with \u22654 high-risk domains demonstrated higher prevalence of multimorbidity, including hypertension, dyslipidemia, and prediabetes.Both patients and clinicians reported that the IRS supported personalised discussions and increased awareness of modifiable risk factors.
