Many patients present to family physicians with symptoms rooted in their social context, lack of purpose, and limited support networks. Drawing on Viktor Frankl\u2019s Man\u2019s Search for Meaning, this workshop highlights how purpose, belonging, and motivation influence health outcomes. Social prescribing, integrated with lifestyle medicine, provides a practical clinical approach to address these determinants. Link workers are essential in this process: they bridge healthcare and the community, personalize support, and maintain patient engagement, reducing GP workload and enhancing adherence.Participants will be able to: 1. Explain the synergy between social prescribing and lifestyle medicine in chronic disease management. 2. Recognize loneliness and loss of meaning as clinical issues. 3. Apply the ASSESS model to initiate social prescribing conversations. 4. Develop action plans integrating lifestyle advice, community resources, and potential referral pathways. 5. Identify strategies to overcome implementation barriers in primary care. 10 min \u2013 Introduction &amp; Setting the Scene Short presentation illustrating how social prescribing and lifestyle medicine complement each other. A clinical case (e.g., diabetes, isolation, poor diet) demonstrates how meaning and motivation shape outcomes. 25 min \u2013 Interactive Core: The ASSESS Model in Action Small groups (3\u20134 participants) work through a structured case study. They apply the ASSESS model\u2014linking lifestyle goals with social prescribing options\u2014and consider when and how link workers provide essential navigation and follow-up. 20 min \u2013 Plenary Sharing &amp; Synthesizing Solutions Groups share insights and barriers such as resource limitations or time constraints. Discussion focuses on how link workers reduce these barriers and sustain patient engagement. 5 min \u2013 Wrap-Up and Resources Summary of key principles and distribution of a one-page ASSESS model handout.
