{"id":20710,"date":"2026-07-31T10:33:14","date_gmt":"2026-07-31T10:33:14","guid":{"rendered":"https:\/\/woncaeurope2026.org\/sessions\/improving-hypertension-care-in-two-rural-communities\/"},"modified":"2026-07-31T10:33:14","modified_gmt":"2026-07-31T10:33:14","slug":"improving-hypertension-care-in-two-rural-communities","status":"publish","type":"wsa_session","link":"https:\/\/woncaeurope2026.org\/fr\/sessions\/improving-hypertension-care-in-two-rural-communities\/","title":{"rendered":"Improving hypertension care in two rural communities"},"content":{"rendered":"<p>The project aimed to improve the care and continuous monitoring of patients with hypertension, increase their motivation, foster a responsible attitude toward health, and create a long-term monitoring system in two rural villages of XXX (V1 and V2).Medical records in V1 revealed 45.4% of hypertensive patients had not visited the Center in three years. In V2, hypertensives were not registered in the Health Information System and did not receive necessary monitoring or treatment. The population lacked sufficient awareness about early signs and consequences, and many only visited a doctor when symptoms and complications arose. The family physician collaborated with nurses and directors of the health centers. Active identification, registration, and periodic monitoring of patients was implemented. Patients were invited to the respective Center, blood pressure measured, patient education provided, data recorded, and medications adjusted. Obstacles included lack of motivation among patients and lack of trust towards doctors. In V2 the existing records did not reflect reality.According to WHO data, more than 7.5 million deaths occur globally yearly due to hypertension. A similar problem exists in almost all regions of XXX. This project ensured the development of a system for proper follow-up of hypertensive patients.Hypertension management requires not only pharmacologic management but also continuous communication\/coordination, teamwork, and a systematic approach for data recording and analysis. Attention and individualized care make patients more motivated and responsible. For sustainability, patient visits will be organized every three months to prevent complications; awareness activities will be carried out; and data will be recorded for continuous monitoring. The experience will be shared with other healthcare facilities for overall improvement.In V1, over 4 months, out of 50 previously uncontrolled patients, 23 visited the center, received consultations, medication adjustments, and follow-up monitoring, with all achieving control. In V2, over 3 months, 142 patients were screened, 56 hypertensives were identified (from a baseline of zero), received appropriate counseling and treatment.Patients&#039; attitudes were positively impacted, their trust increased, and health improved through continued collaboration.
