The implementation of these tools has demonstrated greater benefits compared with conventional care for several highly prevalent chronic diseases in primary healthcare settings, such as hypertension and diabetes. The development of projects that promote closer surveillance, combined with health education, and that foster multidisciplinary collaboration between hospital services, primary care, family physicians, nursing teams, caregivers, and patients, may be decisive for improving access to care and enhancing disease control. Remote monitoring thus emerges as an essential tool for strengthening care pathways, and its integration into primary care remains a path yet to be fully explored.1. Acquire knowledge about the state of the art of telemedicine tools 2. Identify clinical situations that may benefit from telemedicine implementation. 3. Understand the key steps required to design and implement a telemedicine project in a primary healthcare center.1. Short introductory presentation: Summary of the current scientific evidence on telemonitoring in primary healthcare. 2. Example of an implementation protocol 3. Jigsaw activity: a) A cooperative learning technique in which students become \u201cexperts\u201d on a specific component of a topic and then teach their peers in mixed groups. b) Each initial group will discuss the implementation of a telemonitoring protocol for a specific clinical context. c) Participants then move to mixed groups to share their initial conclusions and receive constructive critiques from peers who explored other clinical contexts. d) Finally, participants return to their original group to integrate insights and refine their initial proposal. 4.
