Vitamin D deficiency is highly prevalent in this group. However, its direct correlation with balance disorders, muscle strength, and fall risk in primary care settings remains a subject of ongoing clinical debate.To evaluate the relationship between serum Vitamin D levels, balance alterations, functional capacity, and the incidence of falls in community-dwelling people over 65 years of age.A descriptive, observational study was conducted with participants aged &gt;65 recruited from five primary care centers. We collected sociodemographic, clinical, and functional data. Key assessments included balance tests (unipodal test, posturography), the Short Physical Performance Battery (SPPB), and muscle strength measurements. Serum 25-hydroxyvitamin D levels were analyzed, considering levels \u226520 ng\/ml as adequate. A prospective follow-up of falls was performed over a 12-month period to record new events. Statistical analysis included descriptive and comparative methods using the Chi-square test (RStudio 4.3.0).A total of 922 individuals participated. 60% presented adequate Vitamin D levels, 32% had inadequate levels, and 8% showed deficiency. Statistically significant associations were observed between Vitamin D levels and sex (p = 0.0047), performance in the unipodal test (p = 0.0181), and habitual physical activity (p &lt; 0.0001). However, no significant associations were found with posturography (p = 0.265), SPPB score (p = 0.350), extensor strength (p = 0.167), previous history of falls (p = 0.477), or new falls recorded during the 12-month follow-up (p = 0.110).Adequate Vitamin D levels correlate significantly with better performance in the unipodal balance test and higher physical activity, suggesting a potential role in preserving specific motor functions. However, contrary to some expectations, Vitamin D levels alone were not predictive of falls or other functional test outcomes in this specific cohort.While Vitamin D is associated with specific motor function indicators like the unipodal test, it does not appear to directly influence fall risk in this population.
