Menopause is accompanied by unfavorable metabolic changes; however, population-based evidence examining the relationship between menopausal status and steatotic liver disease (SLD) remains limited.To evaluate the association between menopausal status and the prevalence of SLD in middle-aged women.Data from 15,106 women aged 40\u201359 years were analyzed using a nationally representative health survey (XXX). Women with a history of hysterectomy, hormone therapy use, or missing key data were excluded. SLD was defined as a hepatic steatosis index \u226536 in the presence of at least one metabolic abnormality. Menopausal status was determined by self-report. Weighted multivariable logistic regression analyses were conducted to assess the association between menopausal status and SLD, adjusting for age, body mass index, waist circumference, and major metabolic risk factors. Subgroup analyses were performed to examine potential effect modification.Overall, 31.5% of participants were postmenopausal and 22.1% had SLD. Postmenopausal women exhibited a higher prevalence of metabolic risk factors and SLD compared with premenopausal women. After multivariable adjustment, menopause remained independently associated with SLD. The association was strongest among women aged 40\u201349 years.These findings suggest that menopausal status contributes to SLD risk beyond traditional metabolic risk factors. The stronger association observed in younger middle-aged women may reflect the metabolic impact of early estrogen deficiency during the menopausal transition.Menopause is associated with an increased likelihood of steatotic liver disease in middle-aged women.
