CKD is characterized by a progressive decline in renal function to end-stage kidney disease requiring renal replacement therapy, and is associated with reduced quality of life, increased cardiovascular risk, and a rise in healthcare expenditure. This study examines the integrated management of CKD between general practitioners and hospital nephrologists in the XXX, with particular reference to the Regional Diagnostic, Therapeutic and Care Pathway\u00a0 introduced in 2019 and the most recent K-DOQI guidelines published in 2024.The recent definition and classification of CKD, including the emerging role of cystatin C clearance as a diagnostic and prognostic tool, highlight the importance for continuous updating and strengthening of integration between general pratictioners and hospital specialists.The study underlines the impact of innovative therapies, such as SGLT2 inhibitors, the new medication used to treat high blood potassium, biologic drugs for primary and secondary glomerulopathies, as well as new treatments for anemia and hyperparathyroidism. The analysis of regional epidemiological data demonstrates a significant potential for the implementation of these therapies, with clear benefits in terms of reducing disease progression and improving quality of life.I propose operational strategies to enhance collaboration between hospital and primary care, improve access to innovative treatments, adopt\u00a0integrated care models based on the latest scientific evidence to improve the disease progression and patients\u2019 quality of life, and optimize the use of healthcare resources at the regional level\u00a0and develop integrated digital monitoring systems, with the aim of optimizing CKD management.The treatment of CKD requires a multidisciplinary and flexible approach.
