Multifactorial interventions targeting physical, environmental, and medical risk factors are effective in reducing falls.To synthesize recent evidence on fall prevention strategies for adults \u226565 and provide guidance for clinical and community implementation.A literature search was conducted in PubMed in December 2025 using the keywords \u201cPrevention of Falls\u201d and \u201cOlder Adults,\u201d limited to studies published within the past five years and involving adults aged 65 years and older. Randomized controlled trials, cohort studies, meta-analyses and systematic reviews were included.Exercise and balance programs (e.g., balance-focused training using Huber stabilizer and C-mill simulators, gait-based balance exercises, Tai Chi)improve strength, gait and postural control, reducing fall risk.Environmental interventions such as ,nonslip socks and home hazard modifications, enhance safety in community-dwelling older adults.Risk prediction tools (nomograms for frail and pre-frail adults) allow individualized prevention strategies. Medical factors, including vision impairment and comorbidities, contribute to falls, highlighting the need for comprehensive evaluation. Healthcare engagement, including annual wellness visits, improves fall prevention and reduces fractures. Family-centred, nurse-led home modification interventions have been associated with reduced falls in community-dwelling older adults.Multifactorial interventions combining exercise, environmental modifications, and risk-based medical management are most effective.Primary care physicians, who are able to assess individuals together with their family and home environment and maintain continuous contact with patients, are in a unique and strong position to identify risks and provide preventive strategies and counselingIndividualized, multifactorial strategies effectively reduce falls among older adults.
