Yet many primary care teams lack practical tools to translate this knowledge into everyday practice and system-level change.Describe\u00a0key challenges and inequities in the care of older people with multimorbidity in their own context, linked to the themes of Liberty, Equality and Fraternity.\u00a0 Identify\u00a0a small set of feasible\u00a0process measures\u00a0(e.g. continuity, follow\u2011up, care planning) and\u00a0outcome measures\u00a0(e.g. perceived safety, avoidable hospital use) relevant to their primary care setting.\u00a0Draft\u00a0a simple, context\u2011specific\u00a0improvement plan\u00a0for older people with multimorbidity, including roles for patients, primary care teams, and other sectors.\u00a0Share\u00a0and reflect on strategies for interprofessional and cross\u2011sector collaboration (Fraternity) that respect patient autonomy (Liberty) and promote fair access and outcomes (Equality).Methods:\u00a0Short presentations, small\u2011group work, structured discussion, and real\u2011world examples from participating countries. Proposed schedule (90 minutes):  Brief presentation of key evidence on multimorbid older adults: continuity, avoidable hospital use, and patient experience.\u00a0 We will discuss *Shared decision\u2011making and deprescribing in multimorbidity.\u00a0 *Access to primary care, follow\u2011up, and preventive services.\u00a0 *Interprofessional collaboration and territorial organization.Have a clearer understanding of how the themes of\u00a0Liberty, Equality, and Fraternity\u00a0can be operationalized in everyday care for older people with multimorbidity.\u00a0 Gain\u00a0concrete examples\u00a0of process and outcome indicators (clinical, organizational, and patient\u2011reported) that are feasible to collect in primary care.\u00a0 Build connections with colleagues from other countries who are working on similar challenges, laying the groundwork for future collaboration and comparative projects.Caring for older people with multimorbidity is a shared European challenge that sits at the heart of family medicine. By systematically applying the principles of Liberty, Equality, and Fraternity, primary care teams can redesign local systems to improve continuity, perceived safety, and the appropriateness of hospital use.
