A GP-led at scale provider has pioneered NHS England\u2019s \u2018Neighbourhood Health\u2019 model that brings together general practice, community services, mental health, social care, and the voluntary sector to work as one team around local populations. Grounded in the principles of prevention, integration and empowerment, the model offers a practical framework for translating population-health ambitions into daily general practice.Participants will: 1.
