Although general practitioners (GPs) routinely address lifestyle factors, these\u00a0behaviours\u00a0are strongly influenced by social determinants of health (SDH), including income, housing,\u00a0education\u00a0and social isolation. Interventions that\u00a0fail to\u00a0consider these determinants are often ineffective and may increase health inequalities.\u00a0 Nearly 40\u00a0years after the Ottawa Charter, many GPs still ask a pragmatic question:\u00a0what can we\u00a0actually do\u00a0in daily practice to address SDH?\u00a0Across Europe, innovative primary care models are\u00a0emerging\u00a0that go beyond the consultation room. This workshop explores concrete examples, including &#039;health care mediation&#039; programmes\u00a0in France, &#039;social prescribing&#039; initiatives in Spain,\u00a0the\u00a0\u2018medical houses\u2019 model\u00a0in Belgium and &#039;innovative community outreach&#039; initiatives in Quebec.Improve understanding of SDH and their impact on chronic disease management\u00a0 Share practical and transferable primary care models addressing SDH\u00a0 Encourage reflection on the evolving social and community role of GPsThe workshop will use an interactive, participatory format. Participants will be divided into four small groups, each\u00a0analysing\u00a0a real-world European case study (as cited in introduction).\u00a0The theoretical concepts of SDH,\u00a0impacts\u00a0on care pathways of\u00a0chronical\u00a0conditions, etc. will be addressed in an experiential way.\u00a0Participants will gain an in-depth understanding of how the strategies presented can or cannot influence the SDH.\u00a0Moderators will\u00a0facilitate\u00a0discussion around feasibility,\u00a0transferability\u00a0and challenges in routine care. A plenary session will allow groups to share insights and practical lessons.\u00a0 5 min\u00a0\u2013\u00a0Welcome\u00a0and\u00a0presentation\u00a0of the topic\u00a0 35 min\u00a0\u2013 Small-group case\u00a0study\u00a0analysis\u00a0and discussion\u00a0 20 min\u00a0\u2013 Plenary feedback, shared\u00a0discussion\u00a0and exchange of experiencesParticipants will leave with concrete examples, practical ideas and increased confidence to\u00a0understand\u00a0the application of\u00a0social determinants within their own practice settings, despite time and\u00a0organizwithin\u00a0their own practice settings, despite time and\u00a0organizational\u00a0constraints.Addressing social determinants of health is an essential\u00a0component\u00a0of primary care.
