Population ageing increases the number of frail older adults with advanced illnesses where hospital care offers limited benefit. Primary Care is often the only service able to provide continuous, person-centred end-of-life care. This case describes a home-based palliative approach for a 94-year-old woman with probable glioblastoma, highlighting symptom control, family support, and palliative sedation.
