Preventing Loss of Autonomy in Primary Care: A Workshop on Implementing the WHO ICOPE Program

Since 2017, the WHO’s Integrated Care for Older People (ICOPE) program has promoted a comprehensive approach to healthy aging. With a strong focus on prevention, it aims to preserve intrinsic capacities (mobility, cognition, nutrition, mood, vision, and hearing) in adults aged 60 years and older.  Structured into five steps (screening, comprehensive assessment, personalized care planning, follow-up, and community engagement), ICOPE supports clinically meaningful stratification of frailty and functional limitations. Implemented six years ago in the Occitanie region and currently undergoing national piloting in France, the program places primary care and particularly multidisciplinary primary care practices at the forefront. However, organizational barriers persist, notably in the co-construction of personalized care plans, interprofessional coordination, and longitudinal follow-up.- Identify key components for a seamless ICOPE pathway: interprofessional communication, co-construction of care plans, and follow-up. – Share regional implementation experiences, including lessons from Occitanie. – Identify opportunities and barriers for each theme. – Co-develop transferable strategies to strengthen ICOPE implementation across diverse settings.The workshop adopts a collaborative and participatory design,  combining real-time polling (Wooclap), small-group work to identify barriers and opportunities across themes, and plenary synthesis : 1. Introduction and interactive overview of ICOPE, followed by presentation of implementation lessons from Occitanie (10 min). 2. Three thematic group discussions (15 min each), each followed by a plenary feedback session (10 min each):  – Interprofessional communication – Co-construction of personalized care plans – Follow-up and longitudinal evaluation 3. Final synthesis and outlook (5 min).Participants are expected to gain an improved understanding of organizational challenges related to ICOPE implementation in primary care. They will identify transferable lessons from regional experiences and contribute to a collective synthesis of actionable strategies and levers to support broader integration.This workshop is grounded in peer learning on integrating ICOPE, a WHO program designed to prevent loss of autonomy in adults aged 60 years and older, into primary care. It seeks to support wider adoption, uptake and specific adaptation of the program across participants’ regions.
