Health Intervention for Patients with Increased Cardiovascular Risk

Cardiovascular diseases (CVDs) remain the leading cause of morbidity and mortality worldwide, yet a significant proportion of cardiovascular risk is preventable through targeted lifestyle interventions and patient empowerment. This workshop series, “Health Intervention for Patients with Increased Cardiovascular Risk,” is designed to support individuals identified as high-risk due to hypertension, dyslipidemia, pre-diabetes, obesity, sedentary lifestyle, or a family history of cardiovascular conditions and other factors, including high Lp(a), indicated by the ESC. The health intervention presented during the workshop is a part of targeted approach used within the "My Health" prevention program in Poland.The workshop aims to equip participants with the knowledge, skills, and motivation necessary to provide adequate health intervention toward patients to facilitate sustainable changes that reduce overall cardiovascular risk and improve long-term health outcomes.The workshops will combine evidence-based medical education and practical skill-building. Participants will engage in interactive sessions led by healthcare professionals – family physicians certified in obesity treatment and experienced in lifestyle medicine. Participants of the workshop will be divided into three groups and will collaborate for 20 minutes with each workshop leader. First leader will address the thematic modules covering: understanding cardiovascular risk factors, the role of diet, physical activity and safe exercise planning, stress management techniques, sleep hygiene, and behavior-change strategies grounded in motivational psychology. Second leader will address specific guidelines covering the lifestyle interventions toward high cardio-vascular risk with a special scope on European Countries. The third leader will cover a case study for each group to test the abilities of performing an adequate intervention.The workshop format promotes peer support and group learning, creating a motivating environment in which participants can share experiences, challenges, and successes. By the end of the workshops, participants will have gained a deeper understanding of the cardiovascular risk and acquired practical strategies to lower that risk that can be used in their everyday clinical practice with patients.This workshop aims to improve the role security of participants and not only to educate but also to empower patients to take an active role in improving their cardiovascular wellbeing and long-term quality of life.
