As a young resident doctor, we must give suport on call for emergencies at the hospital. In Spain family doctors are also emergency doctors as well. So imagine being at your first on call and having this request from an old male patient with his lovely wife: We just wanted to check if the cold was progressing properly Doctor.We reviewed the case, not important precedents. As he started with a cold a few days before and needed antibiotics with the suspect of progression to pneumonia, we completed the study asking for analitics and chest X- ray. We kept him under observation while working for other emergencies. The surprising results came houres after.Sadly, the big point of this case is that the patient passed away because debuting with an acute myeloid leukemia.We work for long hours seeing patients in emergencies that don't actually pose a life-threatening risk, and due to the heavy workload, we might miss certain information or requests for procedures. Perhaps in another situation, we would have recommended relative rest and follow-up with primary care for patients with cold-like symptoms. Is every suspicion of pneumonia, therefore, worthy of exhaustive testing? Probably not. But the importance of reporting these cases, in my opinion, lies in keeping us always vigilant when dealing with at-risk patients.In spain this disease affects between 3-5 of 10.000 patients, and only 48% of them survives one year after the diagnosis. The earlier the diagnosis is made, the greater the survival rate.Primary care and emergency doctos can achieve early diagnoses of very deadly diseases. Simply by deepening active listening to the patient and taking a good medical history. Sometimes we dont have to wait for patients to ask how the cold must be progressing if we anticipate with a simple cuestion: how are you feeling recently?
