Balancing values in obesity care: From guideline development to implementation
Lisandte VAN DE LAAR, Janneke VEEZE and Kim STOKKERMANS
Obesity is a major public health challenge, rooted in an obesogenic society and largely originating outside the healthcare domain. The development of a national guideline for obesity care (including pharmacological options) revealed tensions between scientific evidence and broader dimensions of healthcare quality. While randomized controlled trials provide answers on effectiveness and short-term safety of weight-loss medications, other dimensions—such as equity, timeliness, and affordability—require value-based deliberation. To address these complexities, we conducted a comprehensive stakeholder analysis and environmental scan, identifying diverse interests and priorities across healthcare, policy, and patient advocacy. To explore how scientific knowledge can be translated into daily practice within societal constraints. To understand the six dimensions of healthcare quality and their relevance in obesity care. To reflect on value-based decision-making and navigating stakeholder interests in guideline development and implementation.Interactive discussion based on real-world dilemmas encountered during guideline development. Participants will work with stakeholder analysis outcomes, implementation data, and ethical considerations through case-based learning. workshop schedule Introduction: framing obesity as a societal and healthcare issue Presentation: exploring the guideline development and implementation process in relation to the six dimensions of quality of care, particularly access and equity dilemmas Case discussions: taking a closer look at the different stakeholders and their interests Group reflection: balancing the values represented by those interests Wrap-up: implications for practice and policyParticipants will gain insight into the complexity of implementing obesity guidelines in context of societal constraints and conflicting (stakeholder) interests. They will learn how stakeholder perspectives shape policy and practice and be better equipped to navigate ethical tensions and systemic constraints in obesity care.Guideline development for obesity care requires more than scientific rigor—it demands ethical reflection and systemic awareness. In a context of limited resources and increasing demand, healthcare professionals must balance conflicting stakeholder interests and strive towards liberty, equity, and fraternity. This workshop offers tools and perspectives to support that process.
